AGREEMENT/TERMS

PLEASE READ AGREEMENT BEFORE REGISTERING
The execution of this form shall serve to release, hold harmless and forever discharge the BATH LIONS MIDGET FOOTBALL ASSOCIATION, it's successors, assigns, directors, and officers of any and all claims, demands, rights and causes of action of whatsoever kind of nature, arising from, and by reason of, any and all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to property, consequences thereof, which heretofore have been, which hereafter may be, sustained while participating in the program of BATH LIONS MIDGET FOOTBALL ASSOCIATION.  I, the undersigned, assume all risks and hazards related to participation, including but not limited to, the actual program and transportation to and from said activity.  I agree this release shall be effective even if interpreted to be counter to public policy under the laws of the Commonwealth of Pennsylvania.

I assume all financial responsibility of medical expenses for the above named player.  BATH MIDGET FOOTBALL ASSOCIATION assumes no liability or cost in the event of injury or illness of any kind due to participation in any activity sponsored by the Association.  I also attest that a legally licensed physician has certified that the above player is capable of participating in the sport. A copy of certification must be turned in to the above players coach before he/she may participate in the sport.  Mouthpieces must be worn while practicing and playing games.

I assume all responsibility for all equipment, clothing, and/or other material issued by the BATH MIDGET FOOTBALL ASSOCIATION.  I agree to return all equipment in the same condition issued, upon request.  Any article of clothing will be returned clean, not dirty.  I agree to reimburse BATH MIDGET FOOTBALL ASSOCIATION for any equipment lost, stolen, or damaged due to negligence, and/or misuse.

I also agree to pay in full all registration and mandatory fundraising fees established by BATH MIDGET FOOTBALL ASSOCIATION and payable in full upon completion of the registration application.  I understand that no applicant shall be permitted to practice or participate in any program unless all fees and funds are paid in full.  All checks and other promissory notes that are returned to the Association for any reason are subject to a $20.00 fee in addition to any and all charges levied by the bank to the Association.  No child will be permitted to practice and/or participate in a sport in which outstanding funds are due to the Association.

I understand that failure to comply with any of the above conditions, or those later set in sole discretion the BATH MIDGET FOOTBALL ASSOCIATION, may restrict continued participation by the applicant.

As Parent or Legal Guardian, I agree that my child will participate in football/cheering for the BATH MIDGET FOOTBALL ASSOCIATION for the current season.  I further agree that he/she abides by the rules and regulations of this organization and the Lehigh Valley Youth Football League.  By signing below I acknowledge that I have read the above information, and I fully understand my responsibility.  I certify that all information given by me to the BATH MIDGET FOOTBALL ASSOCIATION to be true and accurate.
